One instrument has a camera on it and displays the image on a video monitor while the other instruments have graspers, staplers, suture devices, etc on the tips.  The instruments are manipulated outside the patient like so many joysticks while the surgeon watches the monitor to see what's going on inside the patient.  Aside from specific surgical simulators, video gaming is the most similar activity to this.  Knitting, woodworking model building would not work.
