First spotted by techdude13 on the MacRumors forums, the page for Apple's iPhone Upgrade Program now lists mail-in trade-in kits as an option. If you decide to have a new iPhone shipped directly to your door, Apple will also send along a package with a pre-paid shipping label so you can return that old hunk of junk.
