Apple agrees to settlement in iPod battery class action suit

You know, lots of people complain about the iPod's non-user-replaceable (well, not easily, anyway) battery and its
tendency to just plain crap out after while. Then, there are some people who mount a class action lawsuit over it, and
it turns out they've actually won a settlement. Apple isn't actually admitting any wrongdoing or misrepresentation, but
they have agreed to the settlement, which stipulates that third-generation iPod purchasers who've experienced battery
failure during an eligible one-year limited warranty extension (presumably this adds a year to the warranty you
normally get) can submit a claim for either an iPod replacement or a $50 Apple Store credit (hmm, no brainer!). Those
who purchased a new first- or second-generation iPod on or before May 31, 2004 and who've experienced battery failure
within two years can choose between a $50 credit at the Apple Store (retail or online) or a $25 reimbursement check.
Anyone with a 1-3G iPod purchased before May 31, 2004 who has already paid for battery-related services under the Apple
Battery Replacement Program before June 3, 2005 (hurry!) is eligible for a 50 percent reimbursement. If you qualify as
a class member for any of the above, you'll want to go get some more details (peep the Read link), and you'll probably
be wanting this:

[Thanks to everyone who sent this in.]

weird..I just got a claims for in the mail frmo apple, and i have an original mini...does this mean its coverred? I guess the have my address and info from when i registerred it. does anyone know if i'm good?
Actually, the form that Apple mailed me says you don't necessarily have to have the original reciept. You can submit a copy of the check or creditcard/bank statement showing the purchase. You are allowed to black-mark out your account numbers and other purchases in the case of the creditcard/bank statement.
I'd say your bank can easily produce a bank statement for the time period for when you bought your IPod.
Wait, does Apple send you a claims form on its own steam? I was given an original (I think) mini in June '04, a year ago. So how do I get Apple to send me a claims form????
Has anyone who mailed in their claimed received their credit yet? Or, any confirmation of submitting a claim or any info at all?
Hey does anyone know how difficult it is to replace a 3rd generation i-pod, if I bought a kit for $31.00????
also does anyone know if I registered my i-pod but don't have a receipt, received as a gift and they paid by cash, if I can still mail in a claim form??
Thanks.

I sent in my claim form about a month ago.
I think the deadline is sometime in September. Nowhere in the settlement did I read that you needed a receipt or other proof of purchase.
I selected the $50 credit to the Apple Store because I want an Airport Express.
