Ragefire was revamped with new story stuff, but doesn't (yet) have a heroic version. Blizzard hasn't said that they're doing it, but maybe with the Garrosh patch...

Shadows are a huge GPU hog, along with view distance and anti-aliasing/multisampling. Adjust those until you got an acceptable framerate. If you can possibly stand it, lowering your resolution helps a LOT, too.

Titan's Grip animations are based on the one-handed version of the weapon in question, so you won't see pandaren holding both weapons reverse-style.

If you're talking about the old world, it's possible it's a side effect of upped spawn rates from cross-realm zones. In Pandaria, there are a lot of rare mobs, and they're pretty tough, so the spawn rate may be a little higher to compensate for the trouble.
